The City of Chicago has moved to an electronic EDS. You are required to register and complete the disclosure process on-line. You can find it at Online Economic Disclosure Statement (EDS).
After you have completed the online EDS, please print the “Certificate of Filing” and bring it with you for your meeting as instructed by your Family and Support Services' Contracts Unit representative.
